Student affairs offices continually develop, revise and use policies and procedures relating to campus events, enforcement of judicial policies, residential housing, student records, and campus organizations. Equally important are the forms and procedures to implement policies in areas such as residential life, managing campus conflict, enforcement of student codes, risk reducing strategies for campus and off-campus events, student records, and monitoring campus activities and events.

This manual provides edited user-friendly and practice-tested forms for the implementation of student affairs policies. These forms will facilitate administrators reviewing implementation options and in editing and developing new forms.

  • Residential Housing
    Comprehensive leases including residence hall rules, meal plans and residential living obligations to the more focused lease on room assignments and obligations of the student.

  • Disability Compliance
    Documentation of student requests for disability accommodations, complaints about disability responses, and the documents required to complete investigation are provided to enable institutions to improve and assess their program of disability compliance.

  • Judicial Proceedings
    Forms to document and record outcomes of judicial processes including notice of charges, hearing decisions, disciplinary sanctions, and appeal rights.

  • Conflict Management
    Managing campus conflict is an increasingly common function and requires continual reassessment of alternative dispute options. Forms to implement mediation, grievances, ombudsman complaints, and arbitration options are provided.

  • Student Activities
    For implementing policies on student activities, forms for registration of student organizations, for events, for use of campus facilities, and for transport are provided.

  • Waivers and Releases
    High risk areas necessitate the institution take action to reduce its exposure through releases. Releases and waivers covering areas such as field trips, participation in recreational activities, and study abroad programs are included.

  • Student Records
    Students withdraw, waive their privacy rights, and seek access to certain institutional documents. These forms address student records issues.
